The 29-year-old came on after 66 minutes in place of Ivan Rakitic, and scored his team's fifth goal in the 86th minute of the game.
The Turkish player prostrated for a moment in gratitude after his first goal.
He also tweeted a special message to the fans after the game.
Meanwhile, Barcelona broke Madrid's record of nearly three decades ago in this match and become the first Spanish club to play 35 consecutive matches without a loss.
